- Add ?metadata=true support to GET /api/files/{id} to return JSON metadata

Clean Architecture improvements:
- Domain layer no longer depends on serde framework
- Persistence concerns isolated to infrastructure layer
- TokenClaims in auth_service.rs is only exception (required for JWT)
